Position Summary

The Licensed Practical Nurse (LPN) will provide direct patient care and nursing support services to veteran residents in accordance with facility policies, standards of practice, and Department of Veterans Affairs requirements. The selected candidate will work closely with physicians, registered nurses, and interdisciplinary healthcare teams to ensure the delivery of high-quality patient-centered care.

Responsibilities

The LPN will:

• Provide direct nursing care to residents in accordance with established nursing standards and facility policies
• Monitor, assess, and document patient conditions and report significant changes to the appropriate healthcare providers
• Administer medications and treatments as prescribed and within the scope of practice
• Assist with activities of daily living and resident care needs
• Maintain accurate and timely medical documentation
• Collaborate with physicians, registered nurses, and interdisciplinary team members to support optimal patient outcomes
• Ensure compliance with infection control, safety, and quality assurance standards
• Adhere to all facility policies, procedures, and regulatory requirements

Minimum Qualifications

Qualified candidates must:

• Hold a current, unrestricted Licensed Practical Nurse (LPN) license in any U.S. state, territory, commonwealth, or the District of Columbia
• Maintain current Basic Life Support (BLS) certification
• Be eligible to work in the United States and meet all applicable federal employment requirements
• Demonstrate excellent communication, interpersonal, and patient care skills

Applicants must be able to provide:

• Current resume or curriculum vitae (CV)
• Current LPN license
• Basic Life Support (BLS) certification
• Government-issued photo identification
• Professional references upon request
• Additional credentialing documents as required by the facility
